Accident Investigation Course Outline

Course identification, length and regulatory drivers: The following is a workshop outline to conduct management accident investigation training, annual review of the loss history and corrective actions. The specific regulatory driver for this class is 8, CCR 3203 (a) (5) "Supervisors to familiarize themselves..."

Class length: 2 hours

Attendance prerequisites: None

Target audience: Persons responsible for conducting accident investigations, determine corrective action, or control losses.

Training location space & equipment requirements: classroom setting, white board, backdrop, power point projector, and laptop. This equipment can be provided by Du-All Safety.

Learning objectives: To understand the requirements, terms, and concepts associated with conducting accident investigations and applying appropriate corrective actions. Review the annual loss history claims, investigation reports, and specific corrective actions.

Reference and instruction materials: Include loss run reports, class syllabus, and sample forms.

Evaluation instrument(s): Standard evaluation form and 10 question test (Optional)

Refresher training frequency: Recommended annually.
